    Titan didn't kill the Joker. Creutzfeldt-Jakob Disease did.

    Warning: I'm no neurologist, just a video game nerd trying to untangle the science of Batman.

    So the two foremost diseases in the Arkham series always bothered me. First, we've got the Titan Disease, a deadly illness that infects Joker out of nowhere and just... kills him. It's bizarre because it only ever affects Joker, Ivy, and Batman; as far as I can remember, none of the bajillion clone boss fights against Titan goons see the same thing happen.

    Then, we've got "Mad Clown Disease," the disease that turns people into Joker clones. This is waaaay more prevalent, infecting a whole bunch of people - including Batman. Their skin gets pale, their minds go kaput, and for all intents and purposes, they all become a bunch of bad Joker knock-offs. But that doesn't make sense either. How does blood change someone's personality? Is Joker's DNA so dominant that it somehow overtakes other people and transforms them into… him? That doesn't sound remotely scientific.

    So what if that's not what happened?

    During AK, Batman mentions that he's determined the Joker virus to be a strand of Creutzfeldt-Jakob Disease. It's a throwaway line that I ignored during my first few playthroughs because it just sounded like science-y gobbledygook that didn't actually matter to the story. But now that I've done a little bit of research, I'm pretty sure this is actually the key to this game's bioscience and hugely important for the plot of the game.

    Ok, so what is this disease? Before I go much further, know that I'm suggesting what Batman suggests; this is a unique strand of the disease. Not all of our symptoms are going to be present in the same that they normally are… but man do they line up all the same.

    1. Dementia: This is definitely an issue for Joker, even if he'd never admit it. He tells several Arkham psychologists several different backstories, famously saying he prefers his origin story to be "multiple choice." However, Strange and others muse that perhaps he simply can't remember where he came from, and that makes sense. He's obsessed with Batman after the Caped Crusader boots him into a chemical vat… and maybe that's because it's all he can remember. His history before that could be a haze. That would explain why he's so attached to Batman; he's basically the Joker's parent!
    2. Impaired Judgment, Memory, and Thinking: Um… yeah, do I need to explain this one? Joker's sense of morality and basic code of conduct is so absurdly volatile that even his own girlfriend has no idea how to predict his insanity. Check that box off.
    3. Insomnia: Would anyone even notice? He spends night after night facing off against Batman, and presumably, to pull off the stunts he plans, he'd have to spend all day plotting, too!
    4. Involuntary Muscle Spasms: Again, would anyone notice? Then again, Joker quips multiple times about "accidentally" pulling the trigger or pushing that latch release or the like. What if it's not a joke? It's more than possible that he actually did it by accident and is just laughing it off with his impaired judgment. Might need more research on this one.
    5. Loss of Movement, Sight, and Speaking Ability, eventually leading to death: This sounds pretty similar to what we see from Joker in AC. He can't get up and get around like he used to, his voice is a raspy mess, his eyes are glazed over… It's not just a set of general drug-induced symptoms. It's pretty literally the end of the road for patients with CJD.

    Normally, this shows up when people are in their 60s and kills them in about a year. However, for some unique cases, it shows up in people who are much younger. For those individuals, the symptoms show up much sooner and last longer before the patient finally succumbs. And what's more? Those symptoms are primarily related to psychosis.

    To me, this makes way more sense than a mysterious "Titan disease." All the symptoms are there, and it's not unthinkable to suggest that Titan still could have triggered the negative onset of physical symptoms - who knows what that shit does to your body. If it wrecked his immune and muscular system, it makes sense that he wasn't able to fight off CJD nearly as effectively. He wasn't fending off Titan; he was fending off a preexisting condition that was simply exaggerated by the super steroid.

    And this actually explains how the "Joker disease" spread.

    CJD has three different strands: hereditary, sporadic, and acquired. Hereditary disease is genetic and not really important for my analysis. Sporadic isn't either; it shows up only when someone has the disease but doesn't show symptoms.

    The last one is what matters for my analysis. Acquired CJD spreads primarily through contact with the nervous system tissue, especially during medical procedures. That fits right in line with the disease contracted by all of our victims in Gotham Hospitals.

    It also explains why so many of the patients are simultaneously like and unlike Joker. It's not that Joker's blood transforms them into Dollar Store Jokers; they're simply convinced it is. Because of this, when they become infected with CJD, they emulate the side of Joker they most identify with. Charisma sees the twisted showman, Albert King sees the unstoppable force, Christina Bell sees the obsession with Batman, and Henry Adams sees the failed genius rising to wicked fame. As their judgment collapses and they're told they've been infected by Joker's blood, they begin toying with the idea that they might become the new Jokers - and so they do. Imperfect, but only because that's the most "Joker" they know.

    Batman, however, knows Joker better than anyone. So when CJD takes hold of his mind and begins changing his judgment - coupled with the antagonizing factor of Scarecrow's toxin - he hallucinates Joker everywhere, his fears take center stage, and he recreates the most accurate Joker any of the patients face.

    Why wasn't Joker diagnosed? Because CJD is already stupid rare, acquired CJD is more so, and CJD in young patients is unbelievably uncommon. This would be a further manipulation of CJD, probably truly one-of-a-kind.

    There's a story point that actually supports this. At the end of AC, we see Batman racked by not-so-hidden guilt over Joker's death, and near the beginning of AK, Joker does bring it up every once in a while. But mostly, Joker harps on the whole "You're failing your family" beat. If Joker is playing on Batman's fear and shame, you would think he'd focus a lot more on "Why did you let me die?" and "You basically broke your rule when you killed me." And yeah, Joker brings it up, but it's far from the focus of his psychological warfare. Why?

    Because Batman knows. We hear it in a dialogue line. He already figured out by this point that it's CJD, not Titan or Joker Disease, that killed Joker. He knows it was beyond his control. Of course, there's some unsubstantiated residual guilt, but it's not that thing he regrets most.

    If you haven't already commented it, you're about to: "But Batman had Titan disease and was cured, then still had CJD; how can Titan Disease AND Joker Disease both be CJD?" And obviously, the only answer could be… I have no idea. Honestly, if you guys have an ideas, I'm all ears. But for now, I think CJD actually answers more questions than it raises, especially compared to two mysterious "Titan" and "Joker" diseases that don't make a ton of scientific sense.
